Roles and Responsibilities
Duties and Responsibilities include but not limited to:
Clinical Care support br>
- Assist in the effective management and coordination of client care across all clinical care department programmes.
- Handle inbound and outbound client communications across multiple CareWorks programmes.
- Contact new, scheduled, or referred clients according to daily workflow lists.
- Coordinate and follow up on client appointments, ongoing care, and referrals with healthcare providers.
- Support clients in understanding their treatment plans, medication regimens, and healthcare visits.
- Remind clients of appointments for blood tests, medication collection, and address any related issues as required.
- Identify and resolve barriers to care, including transportation, financial, or language challenges.
- Provide language translation services for the clinical department and clients as needed.
- Participate in quality improvement initiatives to enhance client care and service delivery.
- Assist in tracking client outcomes, satisfaction levels, and service utilisation metrics.
- Support the clinical team by gathering, organising, and maintaining client data, including medical records, treatment histories, and insurance information.
- Participate in team meetings, case study discussions, and training sessions to stay informed about best practices in client management.
- Assist clinical case managers and care coordinator in monitoring client progress and ensuring compliance with treatment plans.
- Collaborate with clinical case managers, care coordinators, nurses, and pharmacists to ensure seamless, coordinated care.
- Assist in implementing care plans and interventions developed by the healthcare team.
- Ensure clients receive appropriate care, understand their healthcare options, and experience a smooth journey through the healthcare system.
- Contributes to the creation and implementation of procedures that enhance client management and support services.
- Comply with all relevant healthcare regulations, ethical standards, and organisational policies.
- Identify, discuss, and escalate clinical issues to the Clinical Service Manager as necessary.
ounselling and support
- Receive and call all new clients received from case managers or clinical service manager.
- Call all new or previously scheduled clients listed on the workflow for that day.
- Introduce the client to CareWorks’ procedures on the first call. < r>- Ensure that the client is aware of their status on the first call.
- Check who the client has disclosed their status to and when to ascertain where their support system lies (e.g. partner and employer).
- Deliver counselling and emotional support to clients when required.
- Liaise with Case Manager to ensure doctors’ appointments are booked, enrolment forms are distributed, and necessary blood forms and prescriptions are generated. < r>
Adm nistration
- Review daily workflow on the in-house system to identify clients (new or follow-up) requiring contact that day.
- Verify client personal details and gather any missing information during calls.
- Respond promptly to client enquiries via phone, email, or other communication channels.
- Schedule follow-up calls with clients after each interaction, record them in the in-house system, and notify case managers.
- Document all client interactions in the in-house system, ensuring notes capture all discussions for reporting purposes.
- Monitor and manage client communication channels, including phone, email, WhatsApp, and other platforms.
- Maintain comprehensive client records after each interaction, updating profiles on all inhouse systems.
- Ensure client records are accurate and up to date across all in-house systems.
- Perform general office administration tasks as required.
- Undertake any other reasonable tasks assigned by senior management.
- Respecting and protecting the privacy rights of all clients in terms of the provisions of the Protection of Personal Information Act of 2013 (POPIA)
Qualifications and Requirements
Inherent requirements of the position:
South African Nursing Council Registration up to date at all times < r>Multilingual ability in African languages is a critical requirement. Proficiency in Tshivenda, < r>Xitsonga or isiNdebele is preferred
asic clinical knowledge and experience of HIV / AIDS and its treatment would be advantageous < r> bility to follow protocols, procedures, and care plans < r> ood communication skills < r> revious counselling experience would be beneficial < r> emonstrates a non-judgmental and respectful approach towards key populations < r> level of computer literacy enabling the candidate to work with Microsoft Office tools and learn < r>new inhouse software

Enrolled Nurse Assistants
Position infoThe successful candidate will work on a one-to-one basis with patients in the comfort of their homes OR caring for a patient in a facility. Minimum requirements to apply for this position :
Responsibilities- Be part of a multidisciplinary team. Duties may involve care for patients such as: adults and children with tracheostomies connected to a ventilator, elderly patients with dementia/alzheimer care, quadriplegic patients with PEG tubes, and palliative patients at end of life.
- Provide basic care and assist with personal hygiene (washing, dressing, grooming, feeding, mobility support, toileting).
- Administer medications under the supervision of the registered nurse or the family.
- Document the patient’s condition and progress.
- Provide support with activities of daily living (ADLs).
- Monitor vitals of patients and record.
- Perform basic wound care.
- Manage feeds of a tracheostomy patient by means of a feeding pump.
- Observe changes in the patient's condition and record in the daily report.
- Write reports in good English.
- Assess sleep routines of patients and discuss with the registered nurse; care plans must be adjusted accordingly.
- Understand and apply treatment plans set by other team members (Doctor, NSM, or Physiotherapist) to ensure that tasks are completed appropriately.
- Demonstrate good communication skills with supervisors, colleagues, patients, and family members.
- Knowledge of dealing with emergency situations in a home setting.
- Knowledge of Geriatric Care, Paediatric Care, Dementia Care, Post-Stroke Care, and Ventilation Care.
- Knowledge of catheter care (indwelling, suprapubic, intermittent) and prevention of pressure sores in bedridden patients.
- Knowledge of trachea care or experience in units with tracheostomy and PEG patients.
- Knowledge of suctioning of a tracheostomy patient.
- Prevention of infection in all patients.
- First Aid Certificate will be an advantage.
- Diploma as a qualified Enrolled Nurse (Desired Qualification Level and Accreditation).
