Position Summary
The Home Care Enrollment Specialist provides services to families potentially eligible for DSCC Home Care Services (waiver and non-waiver) statewide. The Enrollment Specialist utilizes skills and knowledge acquired from academic training and professional experience as a social worker or in social sciences. The Enrollment Specialist coordinates and facilitates the timely discharge of children potentially eligible for the Home Care Program from the inpatient hospital setting and those who are home and require in-home nursing.
Duties & Responsibilities
- Under the direction of the Home Care Enrollment Management Team, provides an introduction to care coordination services and facilities the application process for in home nursing: Obtains documentation to determine potential eligibility for DSCC programs and services. Explains programs, services, enrollment procedures, and care coordination to eligible families. Assists families in collecting required medical and financial information necessary for Healthcare & Family Services (HFS) to determine eligibility for Home Care Services. Conducts home assessment to determine the safety of the home environment for the child and determine if home modifications are needed. Coordinates all available health care benefits. Assists families as needed as they interview prospective home care providers to assess suitability and monitor provider services. Works collaboratively with hospital staff and other providers to ensure services are ready at the time of discharge from the inpatient hospital setting. Those services include, but may not be limited to, in-home nursing and medical equipment and supplies, and care coordination. Collaborates with DSCC Core/Connect Care team during the enrollment process for potential dually enrolled participants. Elicits health and medical histories; assesses social history and family history of children requesting, or eligible for, DSCC services. Conducts in-person or telephonic comprehensive health assessment. Proceeds with timely data entry to ensure enrollment process compliance.
- Participates as a member of the Home Care Enrollment multi-disciplinary team: Complies with University, Division, Regional Office, and HFS policy and procedures. Provides discipline-based expertise to the Home Care Enrollment multi-disciplinary team. Provides discipline-based expertise to other DSCC teams (Core/Connect Care teams) when the enrollment process takes longer due to case complexities such as DCFS involvement or difficult transition. Complies with state and federal regulations related to children enrolled in the Home Care Program or any subsequent program related to in-home care for children with complex medical needs.
- Promotes interagency collaboration and an organized network of integrated services: Provides community education programs regarding DSCC services. Participates in developing and/or implementing a networking plan for the Home Care Enrollment team.
- Participates in special projects impacting DSCC: Participates in agency committees/projects on a regional or statewide basis. Performs other duties as assigned such as presentations or refreshers to other DSCC teams regarding referral and enrollment criteria. Attends all internal staffing on assigned home care clients, and provides technical assistance Perform other related duties and participate in special projects as assigned.
- Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker.

Three (3) years (36 months) of progressively more responsible full-time experience in social work in a medical/clinical or social service agency setting.

1. High School Diploma or equivalent.
2. Current Basic Life Support (BLS) or Heart Saver (HS) certification from the American Heart Association. If not available upon hire, a BLS/HS must be obtained within 3 months from date of hire.If certification is not obtained within 3 months of hire, the employee may be terminated or employee may apply for another position for which they meet the minimum qualifications.
3. Medical Assistant credential, approved by the Massachusetts Department of Health (ARMA, NHA, etc.).

The incumbent will be expected to do the following:
- Coordinate service delivery and ensure operational efficiency across all COSUP sites throughout the City of Tshwane;
- Attending and participating in quarterly UP, CoT COSUP Joint Working Group;
- Attending meetings with COSUP Project Manager and COPC Research Unit Director;
- Chairing the weekly COSUP management team meetings, ensuring all tasks allocated are executed;
- Support HR processes including recruitment, onboarding and performance management;
- Building strong relationships (staff and clients) and collaborate effectively with cross-functional teams;
- Serving as a point of contact between COSUP project teams, management and external stakeholders;
- Creating and maintaining comprehensive project documentation, plans and reports;
- Overseeing the preparation and processing of invoices, updating of tracking reports and ensuring that files are maintained for due diligence and financial auditing;
- Liaising with stakeholders to identify and define project requirements, scope and objectives;
- Ensuring that all project activities adhere to UP, CoT and project specific policies and procedures.
- Bachelor’s degree in Health Science, Public Admin, Business or equivalent;
- Three years experience as a project administrator or coordinator.

We offer short-term and long-term positions with immediate availability. This job opportunity provides rewarding small town Emergency/Urgent care work in a collegial setting.
The practitioner will work collaboratively with an inter-professional health care team to provide culturally safe urgent care to patients on-site at Ladysmith Urgent Care Centre. Physician must be comfortable with acute management of minor injuries and medical conditions, and familiar with procedures including cardioversion, intubation, casting.
Our competitive pay consists of MOCAP and REEF bonuses (together around $375 per 7.5-hour shift), in addition to MSP fee-for-service earnings. Rural Retention Program's 4.2% fee premium applies to all MSP billings. We can also offer a hassle-free billing option through utilization of our local billing clerk. Total remuneration typically exceeds $,500 per 7.5-hour shift, depending on patient volume and procedures performed.
Embrace the flexibility of being "on-call" like a rural ER physician, coming in when needed and having the freedom to leave during quiet periods. Our prime location between Nanaimo and Duncan provides access to two sets of specialists and support from CDH and NRGH emergency rooms. We offer either 7.5 or 15-hour shifts with no overnight call.
Our modest facility boasts state-of-the-art resources, including a dedicated team of RNs, basic lab work, EKG/TNK thrombolysis, POCUS (Sonosite), CT scans, several chest tube options, X-ray/casting & finger-trap for fracture reductions, slit lamp, procedural sedation, cardioversion, and IV therapy and infusion capabilities. We also receive ambulances from the community.
Mandatory qualifications include current CCFP (EM preferred, but not required), Advanced Trauma Life Support certification or CARE course, current Advanced Cardiac Life Support Certification, must have done Paediatric Advanced Life Support course at least once. We offer trial shifts and buddy shifts to help you acclimate to the site. During buddy shifts, you'll work alongside a regular physician on a weekday morning, seeing patients at your own pace and billing for your services.

May be eligible for additional remuneration in accordance with the Rural Subsidiary Agreement (RSA), including (subject to change):
Recruitment Incentive: up to a maximum of 10,000 (pro-rated based on FTE)
Relocation: up to a maximum of 15,000 (amount based on location relocating from)
Flat fee: 6,508.80
Fee Premium: 4.20%
CME: 2,200/annum year 3-4 and 4,400/annum after 4 years in the community

Our prime location between Nanaimo and Duncan provides access to two sets of specialists and support from CDH and NRGH emergency rooms. We offer either 7.5 or 15-hour shifts with no overnight call.
Our modest facility boasts state-of-the-art resources, including a dedicated team of RNs, basic lab work, EKG/TNK thrombolysis, POCUS (Sonosite), CT scans, several chest tube options, X-ray/casting & finger-trap for fracture reductions, slit lamp, procedural sedation, cardioversion, and IV therapy and infusion capabilities. We also receive ambulances from the community.
Mandatory qualifications include current CCFP (EM preferred, but not required), Advanced Trauma Life Support certification or CARE course, current Advanced Cardiac Life Support Certification, must have done Paediatric Advanced Life Support course at least once. We offer trial shifts and buddy shifts to help you acclimate to the site. During buddy shifts, you'll work alongside a regular physician on a weekday morning, seeing patients at your own pace and billing for your services.

Recruitment Incentive: up to a maximum of 10,000 (pro-rated based on FTE) Relocation: up to a maximum of 15,000 (amount based on location relocating from) Flat fee: 6,508.80 Fee Premium: 4.20% CME: 2,200/annum year 3-4 and 4,400/annum after 4 years in the community
