Treat patients as directed by Physical Therapist. Record daily treatment notes and weekly progress notes per PT Board. Assist in maintaining department. Participate in Patient Care and Rehabilitation Conferences, as needed. Assist with cleaning and maintenance of treatment area. Treat patients per the physician treatment plan. Assist nursing department with training of Restorative Aides. Communicate with supervisor and other health team members regarding patient progress, problem and plans. Participate in in services training program for other staff in the facility. Record treatment changes per policy and procedures. Instruct patient’s families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services. Report any problems with department equipment so that it is maintained in good working order. Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues. Supervisory Requirements The position is not a supervisory position. Qualification Education and/or Experience Licensed as a Staff Physical Therapy Assistant Board of the States. Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care. Language Skills Ability to read technical procedures. Ability to read and comprehend policy and procedure manuals. Ability to effectively present information and respond to questions from managers, coworkers and families. Mathematical Skills Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. Reasoning Ability Ability to solve practical problems. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Certificates, Licenses, Registrations Licensed as a Physical Therapist Assistant in the state. Employee must meet continue education requirements per state practices. Must maintain a license in good standing at all time with the state board. Physical Demands The essential functions of this position require the following physical abilities: Standing and /or walking very frequently. Sitting occasionally. Reaching with hands and arms frequently, pushing/pulling very frequently. Talking and /or hearing very frequently. Tasting and /or smelling very frequently. Physical Therapist
Summary
Provides physical therapy and related services for patients while maintaining positive levels of interaction with facilities and clients in accordance with the principles and practices of physical therapy and within Reliant Rehabilitation guidelines. - Provide consultation and clinical supervision to PTAs.
- Directly supervise PTA and Rehab Tech in skilled tasks during group and individual treatment in a manner that promotes efficient and effective utilization of resources and facilitates the ongoing development of clinical skills in accordance with State Practice Acts, Omnibus Reconciliation Act (OBRA), and Reliant Rehabilitation policy.
- Build a strong physical therapy program
- Enhance clinical expertise, professional and management skills through interaction with managers, therapists and other professional staff, self-study, and other continuing education activities.
- Maintain a valid state license. Maintain awareness of issues related to the profession of physical therapy and the health care environment.

Mineral Community Hospital is looking for a full-time Physical Therapist to work both inpatient and outpatient services. We are a 25-bed critical access hospital with outpatient services attached in a recently updated facility in beautiful Western Montana. Schedules are flexible to help create a good work-life balance. Staff are friendly and welcoming with great access to your providers, as well as hospital and administrative staff. You will be working alongside one PT and one PTA with a patient population ranging from high school athletes, post-op care, chronic pain, and geriatrics.

Prefer one year of related experience; however, new graduates are encouraged to apply.
Duties
The focus of this position is to enhance patient care and safety while working and collaborating positively as a member of the MCH team. The purpose of the physical therapist is to assess patient ability or disability, prevent deformity or disability, relieve pain, develop, and improve body skills, restore function, and reach maximum performance within the patient’s capabilities.
Equipment/Skills
Must be able to relate to and work with ill, disabled, emotionally upset, and at times hostile people of all ages within the facility or on the phone. Must be able to work with the patient’s physician in the application of prescribed treatment, including prompt reporting of significant information that may influence the care of the patient. Must be able to collaborate with those who can contribute to the patient’s progress including patient, family, nursing, and instruct or supervise supportive personnel. Must share knowledge and skills with other services and disciplines concerned with the welfare of the patient. Encourage interchange of information within the health care facility. Participate in policy making and administrative decisions relating to PT and patient care. Will continue to seek new knowledge professionally. Membership in APTA is optional, but supports growth of the profession, and improvement in patient care, code of ethics, and promotes raising the level of health in our community. Position Summary:
Responsible for providing medically prescribed physical therapy to patients in the home in accordance with agency policies. Participates in developing and implementing the plan of care with goals and documentation appropriate to discipline guidelines. Provides supervision as needed to PTA per JCAHO/CHAP, Florida State and Federal practice act guidelines.
Qualifications:
A. BS degree and graduation from an accredited school of Physical Therapy required.
B. Work experience of at least one year as a Physical Therapist.
C. Able to prioritize and manage time effectively and make independent decisions when necessary.
D. Current Florida Physical Therapist License.
E. CPR certification.
F. Current driver’s license with access to an auto and required insurance coverage.
IV. Job Relations:
A. Reports to: Director of Clinical Services
I. Main Duties & Responsibilities:
A. Performs physical therapy procedures, evaluations, resumption of care, admissions, and diagnostic tests on patients for whom these have been medically prescribed and establishes measurable goals.
B. Teaches appropriate treatment procedures and supportive activities to patients and family members.
C. Translates all exercises into functional activities or activities of daily living.
D. Develops needed plan for modifying equipment, appliances, and the physical surroundings in patient’s home.
E. Interprets to the patient and family the implications of the treatment consistent with the actions and wishes of the physician.
F. Helps the family accept responsibility for providing care.
G. Provides families with information, support and encouragement to help motivate them in their progress.
H. Helps individuals and families to accept and adjust positively to physical, mental, and social limitations and identifies barriers to desired outcomes.
I. Evaluates client’s progress towards goals on an ongoing basis, making changes in the therapy program as necessary, under the physician’s direction.
J. Teaches basic principles of good posture and body mechanics in relation to changing needs of individuals in all age groups.
K. Maintains necessary records.
L. Demonstrates rehabilitation procedures and supportive activities on selected patients to nurses responsible for follow-up services.
M. Supervises home health aide if no other health care discipline is going to the patient’s home.
N. Participates in the recertification process and interdisciplinary team conferences.
O. Completes OASIS assessments accurately at appropriate time points.
P. Notes progress towards discharge planning. Communicates with physician and other disciplines regarding discharge planning.
Q. Participates in the agency PI process as evidenced by performing multidisciplinary chart audits, recording and reporting quality improvement data retrieved on a routine basis.
R. Displays willingness to discuss weaknesses and make improvements in job performance.
S. Demonstrates the ability to be flexible with workflow, department needs and work hours.
T. Updates management regarding potential problems or concerns. Also responsible for consulting with clinicians or managers as needed.
U. Maintains an understanding of the company’s scope of services.
V. Understands the infrastructure and how and where to transfer calls in the company.
W. Maintains a professional image.
X. Uses appropriate phone etiquette. Understands the infrastructure of Covenant Home Health Care to appropriately transfer calls within the agency
Y. Promotes a customer friendly atmosphere for all visitors and ensures patient confidentiality at all times.
Z. Participates in accreditation program.
