MAIN PURPOSE OF JOB
The Clinical Care Coordinator will collaborate with various stakeholders, including; patients, healthcare providers, care teams, facilities, and schemes to ensure that all patients who qualify for care coordination are adequately cared for through the entire patient journey. ESSENTIAL MINIMUM EXPERIENCE
Qualified Paramedic/ Emergency Care Practitioner/ Professional Nurse with at least 3-years' prior experience working with patients in either an administrative, clinical or care coordination role

MAIN PURPOSE OF JOB
