THE ROLE

The Community Support Worker (CSW) Social Support assists clients during day trips or short community breaks through the Kites Program.

Please note: Shifts will start either at Bibra Lake (South) or Jandabup (North).

This is a Casual role (Availability between Monday – Friday, 8am to 4pm)

Key Responsibilities

1. Prepare and plan efficient travel routes, proactively managing traffic and time according to the service attendance sheet.
2. Drive Amana Living vehicles (10 or 7-seater) and assist with mobilising and transferring clients within the community.
3. Provide professional, courteous, and safe transportation to clients and staff, adhering to program requirements.
4. Be attentive to and assist with clients’ personal needs during transportation.
5. Ensure clients stay hydrated and nourished, providing assistance as needed.
6. Engage actively with team members to promote client social interaction and enjoyable outings.
7. Respond to additional client care needs and emergency situations.
8. Maintain the cleanliness and tidiness of the vehicle daily.
9. Ensure seat belts and mobility equipment (wheelchairs, walking frames, sticks) are securely positioned.
10. Notify the Service Coordinator of scheduled maintenance or concerns.

This is a professional position in the Special Nutrition Program for Women, Infants and Children (WIC). This position will report directly to the WIC Director.

Licensure as a Dietitian in the State of Florida is required for this position.

1. Provide nutrition assessment including the gathering of anthropometric and hematological data.
2. Provide nutrition and breastfeeding counseling to clients.
3. Assist and guide clerical and technical support staff of the WIC program.
4. Assist in the development of Walton County WIC nutrition education materials.
5. Perform duties at the main clinic site or any WIC satellite clinic.
