A vacancy exists for a Primacy Health Care Nurse (Day Clinic) reporting to Nursing Service Manager. The successful candidate will be responsible for providing essential healthcare services, including preventative care, disease management, health education, and patient advocacy, to promote community wellness and accessible healthcare.

Critical Outputs

• Manage a commercially viable clinic in line with company guidelines

• Provide lifestyle management screening tests.

• Ensure patient information is held securely, confidentially and accurately maintained on an electronic system

• Administration and reporting to ensure efficient running of the clinic as per company policy and processes

• Ensure payment for services received by ensuring controls in place and adhered to

• Manage, monitor and orders clinic services stock and equipment, along with prevention of loss and shrinkage and monitoring cold chain

• Financial and commercial acumen

• Manage medical waste, ensure a safe environment and safety precautions are taken

• Compile professional, stock and financial management reports

Requirements

• Registration with SANC as a Registered Nurse

• 3-year diploma in nursing or 4-year degree

• Certificate in Primary Health Care

• A minimum of 3 years’ practical experience in wound care management

About the Job:

InGP OOH weassess andtreat patients who require urgentunscheduled intervention suitable for treatment by general practice clinicians when the GP surgeries are closed.Our referrals come from either111 , via adedicated health care professional line used byparamedics anddistrict nursing teams or from theAED orMIU .

We currently offer atelephone consultation first system tooffer high quality remote patient management viatelephone andvideo technology where appropriate, but if apatient needs a face to face review orexamination then this is offered at either of our bases at George Eliot Hospital Nuneaton, Warwick General Hospital, Rugby St Cross Hospital, or Coventry Walk In Centre , as well as via home visits where appropriate.

What you'll be doing

As aANP / ACP working inOOH , you’ll betreating patients face-to-face orvia Telephone Triage with theflexibility to choose whether you’d like to do this in one of ourPrimary Care Centres as a base shift, or at thepatient’s ownhome as part of ourhome visiting shifts .

Additional Role Information

The OOH service is open from 6.30pm – 8am Monday – Friday , and 24 hours over the weekends , with various shifts on offer between these times. You will also have the flexibility to choose when you would like to work in order to fit in with your lifestyle.

We provide thishigh quality care via a skill mix ofGP’s andAdvanced Nurse / Clinical and Paramedic Practitioners supported by anexperienced operational team of shiftCo-ordinators ,Assistants ,receptionists anddrivers . Wehave access to basic day timeGP notes viaGP connect , summary care record, special notes and child protection information sharing. Both thelead nurse andmedical lead work in aclinical capacity as well as amanagerial capacity and areapproachable andsupportive . You will also beusing ourelectronic prescribing system.

We are looking to add to our existing Self Employed workforce but can also offer Salaried and Bank employment options.
