Job Summary

This position will ensure that the health and safety of all clients are met. The LPN will assist the Registered Nurse with planning, coordinating, and managing the care services and act within the state and federal regulations. The LPN is responsible for the overall direction, coordination, and monitoring of direct care service delivery under the direction of the Registered Nurse.

Responsibilities
  • Ensures that care specialists perform their duties and responsibilities in a timely, quality manner and reports problems appropriately.
  • Observes for and reports changes in client condition, or any other concerns, to the physician, family, and management in a timely manner.
  • Initiates emergency procedures for clients and reports to appropriate management.
  • Reviews of client care plans developed by the management and consults about client conditions and their treatment.
  • Assists with the implementation of medical orders.
  • Ensures accuracy and promptness in transcribing medical orders.
  • Carries out proper charting practices and procedures.
  • Assembles and operates medical equipment used for client care.
  • Orders, receives, accurately sets up, and administers medications.
  • Attends required meetings, in-service training, and conferences.
  • Works cooperatively with other departments.
  • Assists in the monitoring and evaluation of the job performance of care specialists.
  • Consults with the RN regarding employee performance problems.
  • Schedules for work of care specialists within established guidelines.
  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
  • Apply common sense understanding to carry out instructions furnished in written, verbal, or diagram form.
  • Must attend all staff trainings and meetings as scheduled.
  • On-Call Rotation required.
  • Other duties as assigned.
Qualifications

Requirements:

  • Must be licensed with the State of Wisconsin or Minnesota as an LPN, and must maintain current unencumbered licensure.
  • Previous experience in a related field.
  • Maintains current background check on file with the State of Minnesota or Wisconsin and agencies involved.
  • Must have a valid Minnesota or Wisconsin Driver’s License.
  • Drives company and personal vehicle in a safe manner and in accordance with MN and WI State Laws.
  • Must be able to read, write, and communicate in English.
Compensation

$30 - $33 hourly
