THE ROLE

Our Community Support Workers have the rewarding role of caring for elderly people in their homes, enabling their overall independence.

Your day-to-day workload will be varied from providing personal care (showering, dressing, hygiene), meal preparation and domestic duties, social support, shopping assistance, transport, and pet care. Additional responsibilities can include fitting and removal of pressure stockings, respite and assisting with medication.

We have a permanent part-time opportunity areas throughout Mandurah.

Area of need: Forrestfield, Lesmurdie, Wattle Grove, Kalamunda and surrounding Suburbs

Roster: Monday to Friday 9am – 5pm

Responsibilities
  • Under the direction of CSP Director and/or Clinical Coordinator, provide case management, crisis intervention, and community support services to consumers

  • Effectively present information to management, staff, and other organizational groups
  • Demonstrate positive working relationships with consumers, staff, and other professionals
  • Promote positive consumer relations
  • Provide consultation, education, and counseling services to family/support systems involved with CSP as needed

Requirements

Key to the advancement of the core academic and university missional mandate is the transformation philosophy underpinned by, inter alia, an emphasis on Africanization, decolonization, decoloniality, and centering endogenous /indigenous knowledge systems, Africa’s knowledge systems, civilizations, philosophies, pedagogies, epistemologies and social justice, whilst also embracing proficiency and deep insight and research of other knowledge systems. This position therefore requires an astute academic and leader who not only possesses vast experience as an academic but also has the gravitas and “hands on” experience as an academic leader with the capacity to inculcate these values in all our academic endeavours, including accelerating the 10 Catalytic Niche Areas.

Creation of knowledge and the formation of undergraduate and post graduate students as critical citizens and contributors in the local and global knowledge arena is also key to the work of this portfolio. This position is responsible for eight (8) colleges, two (2) schools, namely, the Graduate School of Business Leadership and the Thabo Mbeki African School of Public and International Affairs and 26 Regional Centres. The portfolio also oversees the five distinctive areas, which entail, community engagement / engaged scholarship, academic programme approval, accreditation and registration services, tuition support and facilitation of learning, teaching and student support and promoting a culture of lifelong learning. These functions provide Primary Academic Support to the whole institution. Equally, the portfolio is fundamental to providing inputs into Unisa’s overall strategy and a focus on accelerating the shift towards becoming a leading Comprehensive Open Distance e-Learning institution harnessing the full benefits provided by the 4th industrial revolution (4IR). The incumbent of this position must have a clear understanding of 4IR and its implications for teaching and learning and engaged scholarship in the changing higher education and global knowledge landscapes.

It is expected that the incumbent and portfolio will provide strategic direction and managerial leadership aligned to the institutional vision, advice and support pertaining to teaching, learning, community engagement (engaged scholarship) and student learning support. The incumbent must strive to ensure that Unisa becomes a leading CODeL university that develops and provides wide-ranging, flexible and technology enhanced programmes within a strong quality assurance framework for accreditation and related matters in a South African and global context. Taking cognisance of the comprehensive nature of our institution and the changing student profile, the incumbent must develop responsive and student-centered strategies including the development of internship models to produce sought after graduates who have the attributes, discipline, knowledge, and competencies to contribute ethically and creatively to their local, continental and global contexts.

The position is to further ensure a quality undergraduate experience achieved through the strategic alignment and synergies between academic colleges, curriculum development, tuition, the professional development of academic staff, tuition support and student support, and assessment in a technology enhanced CODeL environment.

Inherent Criteria
  • A full professor with a minimum of 10 years relevant management experience in Higher Education institutions with at least 5 years at an executive management level.
  • NRF-rated or globally acclaimed scholar will be highly recommended.
  • Knowledge of local accreditation systems is a minimum requirement and global accreditation systems is recommended.

Position Summary

The Home Care Enrollment Specialist provides services to families potentially eligible for DSCC Home Care Services (waiver and non-waiver) statewide. The Enrollment Specialist utilizes skills and knowledge acquired from academic training and professional experience as a social worker or in social sciences. The Enrollment Specialist coordinates and facilitates the timely discharge of children potentially eligible for the Home Care Program from the inpatient hospital setting and those who are home and require in-home nursing.

Duties & Responsibilities

  • Under the direction of the Home Care Enrollment Management Team, provides an introduction to care coordination services and facilities the application process for in home nursing: Obtains documentation to determine potential eligibility for DSCC programs and services. Explains programs, services, enrollment procedures, and care coordination to eligible families. Assists families in collecting required medical and financial information necessary for Healthcare & Family Services (HFS) to determine eligibility for Home Care Services. Conducts home assessment to determine the safety of the home environment for the child and determine if home modifications are needed. Coordinates all available health care benefits. Assists families as needed as they interview prospective home care providers to assess suitability and monitor provider services. Works collaboratively with hospital staff and other providers to ensure services are ready at the time of discharge from the inpatient hospital setting. Those services include, but may not be limited to, in-home nursing and medical equipment and supplies, and care coordination. Collaborates with DSCC Core/Connect Care team during the enrollment process for potential dually enrolled participants. Elicits health and medical histories; assesses social history and family history of children requesting, or eligible for, DSCC services. Conducts in-person or telephonic comprehensive health assessment. Proceeds with timely data entry to ensure enrollment process compliance.
  • Participates as a member of the Home Care Enrollment multi-disciplinary team: Complies with University, Division, Regional Office, and HFS policy and procedures. Provides discipline-based expertise to the Home Care Enrollment multi-disciplinary team. Provides discipline-based expertise to other DSCC teams (Core/Connect Care teams) when the enrollment process takes longer due to case complexities such as DCFS involvement or difficult transition. Complies with state and federal regulations related to children enrolled in the Home Care Program or any subsequent program related to in-home care for children with complex medical needs.
  • Promotes interagency collaboration and an organized network of integrated services: Provides community education programs regarding DSCC services. Participates in developing and/or implementing a networking plan for the Home Care Enrollment team.
  • Participates in special projects impacting DSCC: Participates in agency committees/projects on a regional or statewide basis. Performs other duties as assigned such as presentations or refreshers to other DSCC teams regarding referral and enrollment criteria. Attends all internal staffing on assigned home care clients, and provides technical assistance Perform other related duties and participate in special projects as assigned.

Minimum Qualifications

  • Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.

Based On Institutional Requirements For Each Position

Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker.

OR

Three (3) years (36 months) of progressively more responsible full-time experience in social work in a medical/clinical or social service agency setting.
