Overview
The Clinical Care Coordinator will collaborate with various stakeholders, including patients, healthcare providers, care teams, facilities, and schemes to ensure that all patients who qualify for care coordination are adequately cared for through the entire patient journey. They will be an advocate for their patients and ensure that the care team effectively coordinates patient care from request to post-discharge. They will also be responsible for monitoring and reviewing the patient’s biometrics and immediate wellbeing with continuous record keeping on multiple electronic platforms.

- To provide tele-health support, review virtual patient wellbeing and ensure patient monitoring maintained within required clinical parameters

- ESSENTIAL EDUCATION: Degree / Diploma leading to qualification as a Professional or General Nurse
- Diploma / Degree in relevant healthcare field
- DESIRED EDUCATION: Post-graduate degree in clinical or healthcare management will be an advantage
- ESSENTIAL MINIMUM EXPERIENCE: Qualified Paramedic/ Emergency Care Practitioner/ Professional Nurse with at least 3-years’ prior experience working with patients in either an administrative, clinical or care coordination role
- DESIRED EXPERIENCE: Virtual care and/or home healthcare experience and chronic disease management would be an advantage

Design and implement CPD program

Current registration with SANC as a Registered General Nurse or equivalent qualification.
Postgraduate diploma in Medical/Surgical Nursing: General or equivalent as accredited by SANC
5 years proven work experience in a multi-disciplinary Intensive Care Unit including Cardio Thoracic, Neurosurgery, Vascular, General Surgery and Medical is essential.
Postgraduate diploma in Nursing Education or equivalent as accredited by SANC
Assessorship certified
A working knowledge of Trauma, Midwifery and Paediatrics
Knowledge of scope of practice of all Nursing categories as well as the regulations of the SANC
Previous experience as an educator / facilitator /mentor would be advantageous
Excellent communication skills
Excellent interpersonal skills, organisational and problem-solving skills

About these roles
The Clinical Nurse is responsible for providing ahigh standard of culturally safe, direct patient care within the Mparntwe Urgent Care Clinic. MAIN PURPOSE OF JOB
The Clinical Facilitator plans and facilitates identified training and development interventions to ensure competency of all staff. They can be allocated to a specialist unit (e.g. the ICU) or work across the nursing units of a hospital.

Postgraduate Diploma in Nursing if a Clinical Facilitator in a specialist unit (e.g. Perioperative Nursing) and a certificate of competence in Generic Assessment (Assessor course)
