About Health Coaches

Health Coaches offers a service to promote sustained lifestyle change and support through behavioural change, emotional support, benefit navigation, goal setting and tracking in order to decrease the cost of health care and improve a member’s quality of life.

Key purpose

To effectively coach eligible members in various programs ensuring that:

• The coaching program outcomes and objectives are achieved.
• Members achieve sustainable health and lifestyle improvements in addition to their personal key objectives.

Key outputs

• Applying various coaching techniques to allocated members to ensure program and personal goals are achieved
• Building positive relationships to ensure successful outcomes
• Effective planning and organizing of schedules and sessions to optimize program adherence and completion rates.
• Assist members with benefit navigation, referrals and activations to optimize benefits available to them under their scheme and non-scheme products.
• Health and Lifestyle education, tracking and monitoring to achieve sustainable improvements.
• Educate members on how to manage certain chronic diseases to avoid relapses, hospital admissions and disease progression with the necessary referral to the relevant health care professional and or other therapeutic interventions.
• Accurate tracking, reporting and capturing of coaching sessions.
• Meeting set targets: productivity, quality, customer satisfaction scores.
• Calling eligible members to enroll into the various coaching programmes

• Clinical qualification accredited with a health professions body
• Knowledge of DH and Vitality products, benefits, funding, and policy rules
• Coaching experience advantageous
• Excellent communication skills, written and verbal
• Proficient in MS Excel, Word, PowerPoint
• registered Dietitian or Biokineticist

Our health coaching team is expanding and we are on the look out for dietitians to help us serve our patients. You will be joining our growing team of Health Coaches in South Africa and will work directly with our team in Australia to serve our patients.

As you will be supporting our Australian patients alongside our Australian Health Coaches, initially you will be required to work hours allowing crossover with Australian timezones such as starting at 6am local time (with the opportunity for flexibility in the future). As a Dietitian in the team, you will be responsible for helping us shape the clinical guidance and strategy about how to manage diet and nutrition. In short, the role includes:

• Providing 1:1 and group coaching and advice (with a focus on obesity management)
• Directing patient queries and feedback to the relevant internal channel
• Working in a cross functional and multidisciplinary team to triage, respond and escalate communications from patients to relevant teams to minimise patient safety risks
• Surfacing patient insights to the wider business

In this role you will work with our clinical team, patient operations team and growth teams to ensure our patients receive a high quality of support and care. This role requires a high level of empathy, the ability to prioritise simultaneous activities and communications with multiple patients while ensuring high attention to detail, and a patient-centric-oriented approach. You will be interacting directly with real people dealing with real health issues and medical diagnoses and must be able to identify, respond to, and escalate interactions and situations appropriately in accordance with guidelines and for exceptions.This role will be important to the continued success of Eucalyptus and will filter important information back through the rest of the team so we can continue to improve the health of our patients.

• Dietitian specific: Registered Dietitian (registered with HCPSA: Health Professions Council of SA), 1-3 years of dietitian-led coaching services and completion of relevant ongoing CPD requirements.
• Naturally empathetic, reliable, caring, with strong problem-solving skills
• Extremely organised, independent and can work well in a team
• Excellent communication skills
• Confident using technology and excited by fast-paced change and creative problem solving
• Experience with behaviour change techniques aimed at helping patients achieve weight loss goals.
• Goes without saying - you're obsessive about patient experience and thrive on understanding a patient’s problems and finding the best solution

• Previous experience working in digital health (tele-health)
• Qualifications in exercise science or experience working with patients improving their physical activity levels

This role is hybrid; a blend between working from home and travelling occasionally to visit learners for progress reviews and

The Role

As a Development Coach, you will be delivering and facilitating teaching, training, coaching and mentoring apprentices. The
apprenticeship standard you will be delivering is up to Level 5 Leader in Adult Care.

The role will involve:
- Lead a caseload of learners and support them to achieve their apprenticeship or qualification, as well as any additional
requirements such as a Diploma through to Gateway and successful End Point Assessment
- Regularly assess work and provide constructive feedback including SPAG and update the E- Portfolio system (Aptem)
- Identify and support learners with additional learning needs and successfully use the neurodiversity platform Cognassist to
support this
- Support those learners who require Functional Skills maths and/or English at Level 1 or 2
- Take responsibility to ensure safeguarding, PREVENT, British Values, Health, Safety and Wellbeing and equality, diversity and
inclusion are promoted and monitored continually in all areas of the job role

- Hold or be willing to complete Assessor Award CAVA or TAQA Level 3, A1, D32/33 or equivalent
- Hold or be willing to complete Teaching Qualification, PTLLS or equivalent

The Role
As the Women’s Health Educator, you will be responsible for overseeing, monitoring, and ensuring that evidence-based Women’s Health services are provided within the Central Australia Region. You’ll work collaboratively with a range of service providers to build and maintain capacity of women’s health services in remote communities through clinical support, facilitating accredited training, mentoring, and orientation of staff. You’ll also travel to remote areas regularly by road, light plane, and stay for up to a week as required.

About you
To be successful, you’ll have:

• Current Registration with Nursing and Midwifery Board of Australia or eligibility to obtain.
