Lead assigned patient service lines and therapeutic programmes:
In collaboration with Group Clinical Pharmacist, ensure the adoption of medication utilisation frameworks, standards and system at hospitals
Facilitates pharmaceutical care services, direct patent care programme, medication utilisation systems and care areas to assure medicine utilisation activities are aligned with patient care needs, best practice and regulatory standards
Monitor and review profiles / chart to identify, prevent or mitigate drug related problems, improper medicine use or dose selection, over-indications, medication without indication and treatment failures
Implement stewardship activities and surveillance programmes
Track and evaluate assigned pharmacy programmes at the facility for quality and routinely benchmark against local and international best practice
Ensure clinical pharmacy is an integral part of health-care delivery system by actively collaborating with healthcare providers within the facility
Work within established parameters, protocols and frameworks for medicine use and control
Identify high risk patients and implement interventions to improve quality and safety
Make appropriate evidence based, patient-centred medicine recommendation
Provide pharmaceutical services throughout the hospital facility:
Ensure all transactions are recorded and reported on the approved platforms and systems
Provides accurate, safe and timely medicine therapy based on individual patient need
Follow the established escalation processes pertaining to clinical pharmacy
Ensure recommendations considers cost beneficial initiatives adopted in the group
B. Pharm and / MSc (Med) Pharm or M. Pharm (Clinical Pharmacy)
5 – 7 years' experience in a clinical pharmacy environment
Hospital experience is non-negotiable
Relevant operational experience and application of clinical pharmacy masters in a hospital setting (non-negotiable)

About the Role

Key Responsibilities:
- Prepare Best Possible Medication History lists for hospital inpatients
- Conduct medication chart reviews and medication reconciliation
- Dispense and supply medication and pharmacy products
- Coordinate patient medication requirements for inpatient stay, day infusions and discharge
- Develop and maintain professional relationships with key stakeholders to ensure excellent customer service to clients and patients
- Support patients, carers, hospital staff and associated employees
- Educate and advise staff and customers on health promotion, disease prevention, drug selection and the proper use of medicines
Who are we looking for?
We are seeking a highly driven and team-oriented pharmacist who can deliver a high standard of customer service with sensitivity, compassion, and professionalism. You will have high attention to detail and strong time management and prioritisation skills.
Key Requirements:
- Full general registration as a pharmacist in Australia (AHPRA) or New Zealand (PCNZ)
- Previous dispensing/clinical experience in an Australian or New Zealand based hospital or community pharmacy is essential.
- Previous experience in a clinical setting (highly regarded)
- Have strong knowledge of the Pharmaceutical Benefits Scheme (PBS) (highly regarded)
- Possess effective time management, communication and teamwork skills
- Be flexible and adaptable, working with a diverse team
- Embrace change and commit to ongoing learning
- Value attention to detail and be able to work autonomously
