MAIN PURPOSE OF JOB
The Clinical Care Coordinator will collaborate with various stakeholders, including; patients, healthcare providers, care teams, facilities, and schemes to ensure that all patients who qualify for care coordination are adequately cared for through the entire patient journey. They will be an advocate for their patients and ensure that the care team effectively coordinates patient care from request to post-discharge. They will also be responsible for monitoring and reviewing the patient's biometrics and immediate wellbeing with continuous record keeping on multiple electronic platforms.

KEY RESPONSIBILITY AREAS

  • To optimise the patient experience by coordinating various care plans for several patients at once
  • Reporting, analysis and feedback
  • To provide tele-health support, review virtual patient wellbeing and ensure patient monitoring maintained within required clinical parameters
  • Personal effectiveness

Required Education
ESSENTIAL EDUCATION

Degree / Diploma leading to qualification as a Professional or General Nurse

Diploma / Degree in relevant healthcare field

DESIRED EDUCATION

Post-graduate degree in clinical or healthcare management will be an advantage

Required Experience
ESSENTIAL MINIMUM EXPERIENCE

Qualified Paramedic/ Emergency Care Practitioner/ Professional Nurse with at least 3-years' prior experience working with patients in either an administrative, clinical or care coordination role

Desired Experience
Virtual care and/or home healthcare experience and chronic disease management would be an advantage

ABOUT THE ROLE:

To provide quality care to patients in need of dialysis. Collaborate as a team to ensure effective patient care delivery and achievement of desired patient outcomes.

RESPONSIBILITIES:

The Acute Clinical Technologist will be responsible for:

  • Communication of clinical information with patients and their relatives.
  • Execution of prescribed treatments.
  • Refer newly diagnosed patients to appropriate dialysis facilities in their vicinity, ensuring the receiving clinic is provided with the patient information to ensure continuity of care.
  • Monitor new vascular access for bleeding, bruit and other complications and report to Clinician.
  • Adverse events and deviations from treatment prescriptions to Clinician and line manager
  • Report changes in patient progress effectively.
  • Communication with all staff to ensure patient treatments are carried out according to prescriptions.
  • Monitor and react on incoming lines of communication (email, text messages, voice calls etc.)
  • Inspect equipment for faults and report to technical department
  • Ensuring Acute equipment is being stored correctly and disinfected regularly (plugged in to prevent dead battery)
  • Educate patients and family members.
  • Educate nursing staff in hospitals on management and care of renal patients.

EDUCATION, EXPERIENCE AND CERTIFICATION REQUIREMENTS

  • National Senior Certificate (Matric)
  • Clinical Technologist: Nephrology Qualification
  • Valid SANC / HPCSA registration
  • ACLS and BLS Certification
  • Computer literate
  • Minimum 4 years dialysis experience
  • Must have own transport and valid driver's license.

Forensic Services
team as a
Clinician,
where you will be responsible for:

  • Providing forensic mental health services in a voluntary treatment program that works with youth in relation to Harmful Sexual Behaviour (HSB). Priority is for youth who have engaged in or have been convicted of sexually harmful behaviours. The program works with young people and their families, in metropolitan, rural, and remote regions of SA (via telehealth when required).
  • Providing services at the Youth Court and Kurlana Tapa Youth Justice Centre.
  • Addressing inconsistencies between clinical practice and policy
  • Being responsible and accountable for own professional practice standards and for activities delegated to others
  • Conducting mental health assessments, providing education, counselling, and group work as essential components of care
  • Consulting and liaising with other professional disciplines and nongovernmental organisations to coordinate care
  • Providing direction, support, orientation, and education to other staff using clinical expertise
  • Leading and participating in action research projects, quality improvement initiatives, and policy development within the practice setting

About You
To be successful in this role, you will have:

  • Registered or eligible for registration as a nurse by the Nursing and Midwifery Board of Australia and who holds, or is eligible to hold, a current practicing certificate. Must be enrolled in an approved mental health course or hold a qualification in mental health practice.
  • Demonstrated ability to conduct mental health assessment and therapeutic interventions of a complex nature with children, adolescents with eating disorders and /OR emotional and behavioural difficulties
  • Demonstrated ability to relate to people from a diverse range of cultural and social backgrounds
  • Well-developed interpersonal and communication skills including the ability to work with individuals and families for positive mental health outcomes and to foster productive working relationships across disciplines
  • Demonstrated ability to work and contribute positively in a team environment
