Northwest Colorado Health is seeking a PRN (as needed) Patient Care Representative with the passion and skills to deliver high quality care. We have been providing primary healthcare to residents since 2008. Today, you will join a healthcare organization and a staff of 11 providers at four locations, delivering primary medical, dental and behavioral health care to more than 5,500 patients. You will be welcomed into our caring environment to serve the community in which we live.
We have high expectations for you as a Patient Care Representative. We need you to:
- Be responsible for many aspects of preparing, scheduling, and fee collection of patients and clients in the clinic setting.
- Be responsible for scheduling and collecting financial data and fee collection for clients and patients accessing services at the clinic.
- Provide high quality customer service to visitors, peers, clients, and patients in person, through electronic communication, and on the telephone.
- Perform general office duties as assigned.

What You Will Do
- Direct and coordinate clinical departments and sites; assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations (Conditions of Participation).
- Supervise and provide direction to nursing staff and team members, in an effort to ensure quality, compliance with Plan of Care, assessment and reassessment of patient's needs and continuity of services by appropriate health care personnel.
- Supports employee and patient education to enhance knowledge, skills and achieve quality experiences. Continuously trains nursing staff to retain employees with the highest quality of patient care.
- Provide coaching, development, feedback and annual evaluations to team members in a professional and timely manner.
- Performance of other duties as required

Critical Outputs- Provide a comprehensive pharmaceutical service by evaluating all aspects of patient care to drive evidence-based pharmacotherapy, with the ultimate goal of improving patient care and outcomes.
- Implement all components of the LHC antimicrobial stewardship program.
- Compile and update antibiograms, and monitor microorganisms cultured at unit level. Discuss trends with local microbiologists and build effective working relationships.
- Drive the delivery of key performance indicators in clinical pharmacy to improve patient safety, improve patient outcomes and enhance overall clinical pharmacy service.
- Utilise surveillance system optimally to support effective prioritisation and work practices in wards.
- Preparation for and active participation in internal stakeholder meetings, including presentation of progress and gaps.
- Provide appropriate clinical pharmacy support for cost of care initiatives and projects - drive appropriate implementation.
- Enhance cooperative and influential working relationships with doctors and other functional stakeholders.
- Provide drug information and evidence-based guidance and training for other healthcare professionals.
- Elevate the performance on medication safety management and medication error interventions at ward level.
- Implement customer satisfaction and quality initiatives as identified.
- Comply with all legal, professional and procedural requirements pertaining to the dispensing of prescriptions.
- Participate in the implementation of and compliance with LHC Quality Management Systems;
- Participation in industry conferences, relevant societies and external stakeholder meetings.
- Drive the appropriate utilisation of cost-effective medication and formulary medication in the hospital.
- B Pharm and relevant post-graduate certificate/degree in ward pharmacy/clinical skills, studying towards a certificate/master’s degree in clinical pharmacy, or committed to completing a post-graduate certificate in clinical pharmacy within the next 12 months.
- A minimum of two (2) years of proven work experience in a clinical pharmacy setting.
- Current registration with the South African Pharmacy Council as a pharmacist.
- Understanding of pharmacy and related healthcare industry, legislation, regulation and challenges.

- Drive the delivery of Key Performance Indicators in clinical pharmacy processes and practice, to improve patient safety and outcome

- Pharm is essential, a qualification in a clinical pharmacy skills program would be advantageous
- A minimum of 2 years hospital experience required
- Relevant clinical knowledge and experience in the critical outputs
- Experience within an antimicrobial stewardship team, implementation of antimicrobial stewardship and clinical pharmacy initiatives would be advantageous
- Current registration with South African Pharmacy Council as a pharmacist

Requirements and Experience
- Relevant qualification within a specialist area or equivalent NQF 7.
- Registration with the South African Nursing Council as a Registered Nurse/ Midwife or Registration with HPCSA as a Clinical Technologist (Independent Practice)
- Experience in renal nursing is a strong recommendation.
- Capacity to implement and maintain health practice standards required by accredited bodies and appropriate health legislation.
- Must be able to work under pressure in a constantly changing environment.
