Responsibilities
  • Patient Assessment: Conduct comprehensive evaluations of speech, language, voice, and swallowing abilities using standardized tests and clinical observations. Develop accurate diagnoses based on assessment results.
  • Treatment and Education: Implement evidence-based therapies for speech, language, voice, and swallowing disorders; monitor and adjust treatment plans; educate and support patients and families on disorders, treatment strategies, and daily communication improvement.
  • Documentation: Maintain accurate records of evaluations, progress, and treatment plans, ensuring compliance with legal and ethical standards.
  • Collaboration: Work with healthcare providers to ensure effective patient care and participate in multidisciplinary team meetings as needed.
Qualifications
  • Education: Degree in Speech Language Pathology from an accredited institution.
  • Licensure: Valid state licensure as a Speech Language Pathologist, or license eligible
  • Certification: Current state licensure as a Speech-Language Pathologist. Certification from the American Speech-Language-Hearing Association (ASHA) (CCC-SLP).
  • Skills: Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills. Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Speech Language Pathologist

Summary

Provide Speech-Language Pathology (SLP) Therapy and related services for patients while maintaining positive levels of interaction with facilities and clients in accordance with the principles and practices of Speech-Language Pathology therapy and within Reliant Rehabilitation guidelines. Provide quality patient care including screening and evaluation of patients, development of initial and ongoing treatment plans of care, discharge plan, and case management responsibilities. Maintain professional conduct that represents Reliant Rehabilitation’s commitment to quality and service in all interactions with clients, including positive levels of interaction and rapport with facilities and clients. Adhere to all state and federal regulations regarding speech-language pathology therapy.

Essential Duties And Responsibilities

  • Optimize communication, cognitive-linguistic abilities, and swallowing skills of patients by providing, directing, and/or overseeing speech therapy
  • Build a strong Speech-Language Pathology (SLP) therapy program
  • Enhance clinical expertise, professional and management skills through interaction with managers, therapists and other professional staff, self-study, and other continuing education activities.
  • Maintain valid state license. Maintain awareness of issues related to the profession of speech-language pathology therapy and the health care environment.
  • Understands various relevant payment models related to billing and treatment guidelines, as well as clinically appropriate means/modes of delivery. Provide quality care as well as reasonable goals and outcomes within the guidelines.
  • Be compliant with infection control procedures and environmental safety protocol within a facility

Davis School District is accepting applications for Speech-Language Pathologists for the 2025-2026 school year. GENERAL FUNCTION: Under the direction of the Related Services Director, the Speech-Language Pathologist is responsible for evaluating, determining eligibility, and providing appropriate supplementary aids and services to assigned students, according to Federal and State guidelines. Salary: within District guidelines on Table 1 Essential Job Functions: 1. Administers, scores, and interprets formal and informal speech and language tests including, but not limited to, standardized norm-referenced speech and language tests which are appropriate in determining eligibility for special education services and providing diagnostic information for educational programming. 2. Develops/adapts, administers, scores, and interprets curriculum-based assessments in making educational program decisions. 3. Observes, records, analyzes, and summarizes the speech and language behavior of students referred for evaluation or otherwise indicated as requiring such, in the regular classroom setting and other settings as appropriate. 4. Coordinates, administers, scores, and interprets audiological hearing screenings for schools and communicates the results to appropriate people and agencies. 5. Develops an IEP in collaboration with other multidisciplinary team members, for each qualifying student utilizing information obtained from the completed assessment battery. 6. Develops appropriate lesson plans consistent with the IEP goals and objectives, following a specified plan. 7. Delivers specially designed instruction consistent with the IEP and tiered educational (academic and social) supports, as appropriate, that are aligned with the students needs. 8. Utilizes a system for ongoing maintenance of individual and student progress data for complying with local, state, and federal requirements. 9. Maintains classroom organization and discipline. 10. Establishes and maintains standards of pupil behavior needed to achieve a functional productive learning atmosphere. 11. Utilizes computers and other technological support equipment in assisting students with instruction. 12. Provides and maintains a safe and supervised environment in the therapy setting. 13. Evaluates student growth, keeping accurate records and preparing periodic progress reports to be shared with parents, other staff members, and appropriate medical personnel. 14. Communicates with parents on an ongoing basis through conferences and other means discussing progress and defining and refining individual goals. 15. Communicates, cooperates, and collaborates with other professional staff, in academic, social, and personal growth areas. 16. Supervises Speech-Language Technicians as outlined in the USBE SLT Handbook. 17. Maintains supervising competencies consistent with the USBE SLT Handbook. 18. Demonstrates adherence to the professional Code of Ethics as outlines by the Utah Professional Practices Advisory Commission. Minimum Requirements: 1. Masters Degree in the area of speech-language pathology or equivalent from an accredited college or university. 2. Valid professional educator license for the State of Utah with a Speech-Language Pathologist area of concentration. Knowledge, Skills and Abilities: 1. Strong oral and written communication skills. 2. Strong computer and educational technological skills. 3. Ability to work independently with little direct supervision. 4. Ability to work as part of a team. 5. Ability to establish and maintain a positive learning environment and handle student management issues in a calm manner. 6. Early Intervention Speech Language Pathologist, La Plata, San Juan, and/or Archuleta counties

Community Connections, Inc. is seeking a part-time licensed Speech Language Pathologist to provide SLP services for children birth – 3 in Early Intervention. Flexible schedule and hours may range from 2-16 hours per week. Services are provided in homes and community settings, both in-person and via Zoom/telehealth. We promote a coaching model (empowering parents and teachers to continue implementing strategies between visits). Qualified applicants must delight in working with young children and enjoy being part of a team supporting families. Ability to provide services in Spanish is ideal, but not required. Intimate knowledge of Indigenous (primarily Diné and/or Ute) or Hispanic cultures is also ideal but not required. $35-40/hr DOE (plus mileage) includes direct service as well as prep and travel time. Reports To: Director of Children and Family Services

FLSA : Part-time/Hourly/Non-exempt

Job Description : Early Intervention Therapists serve Community Connections as part-time employees working with children and families through the Part C/Early Intervention Program. Schedules are flexible, and are generally scheduled with families and coordinated with the Child Find team as needed. Participation in needed Part C trainings is required. Must provide own transportation and be willing to travel around local communities to families’ homes, childcare settings and other community locations where children and their families are.

Responsibilities:

  • Participate in public and private insurance billing as requested and as able.
  • Become a Medicaid provider.
  • Assist in developing Individual Family Support Plans (IFSP).
  • Implement assigned strategies and action plans from the IFSP.
  • Work with the Service Coordinator for each family to identify family priorities and gather resources for the family.
  • Participate in trans-disciplinary teams as needed.
  • Attend IFSP meetings and reviews.
  • Provide ongoing assessment, at least annually, with a qualified tool and family input.
  • Maintain current and accurate documentation, including certifications, licensing, endorsements and registrations, and shall register and update his or her information at least annually in the provider database maintained by the division for developmental disabilities.
  • Participate in the reporting of Child outcomes, meeting the minimum requirements.
  • Submit paperwork for billing and payroll in a timely and accurate manner.
  • Stay current in trends in Early Intervention, Best Practices and Evidence-Based Practices for applicable field.
  • Respond to families’ needs in professional and courteous manner, building relationships with families and children that will become the basis for all interventions.
  • Communicate problems or concerns to the Service Coordinator or Early Intervention Program Manager immediately.
  • Fulfill other duties requested by the Early Intervention Services Director to assure community compliance with the State and Part C Federal Assurances.

What You Will Need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required. Speech-Language Pathologist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. Education: Bachelor's degree required. Master's degree preferred. Additional Preferences Bachelors in Speech pathology Required. Minimum of 2 years of geriatrics experiencestrong preferred.   • Provide speech, language, voice, fluency, and swallowing therapy to pediatric clients in a home health setting.
  • Conduct evaluations, develop individualized treatment plans, and set measurable goals tailored to each child's unique needs.
  • Collaborate with families and caregivers to support communication development and carryover of therapy strategies at home.
  • Work closely with our interdisciplinary team to ensure a holistic, family-centered approach.
