Job Description
Cares for oncology patients through the continuum of care. Provides individualized assistance to patients, families and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial care from prediagnosis through all phases of the cancer continuum to promote optimal outcomes. Maintains regular contact with medical staff, physician providers and their office staff and documents key components of patient interactions. Collects pertinent data and information related to identifying outcome measures.

2. *Conducts initial and periodic patient assessments to identify physical, social, emotional, cultural and spiritual needs and barriers to care.
3. *Collaborates with other healthcare professionals to develop a plan and facilitate referrals to address needs and barriers.
4. *Facilitates the coordination of patient care services to ensure excellence in patient care and timeliness of patient care including coordination of services, treatment and communication among the medical team.
5. *Facilitates development, implementation and adherence to clinical and evidence-based guidelines, as appropriate, to ensure optimal clinical outcomes.
6. *Builds therapeutic and trusting relationships with patients, families and caregivers through effective communication and listening skills.
7. *Provides anticipatory guidance, education and psychosocial support throughout the cancer continuum.
8. *Makes rounds on inpatient units and outpatient settings. Contacts oncology patients as appropriate.
9. *Participates in cancer conferences to further care planning and communication with healthcare providers.
10. Assists in achieving and maintaining American College of Surgeons (ACOS) certification/accreditation through meeting or exceeding Center of Excellence standards of care.
11. *Documents patient encounters and provided services and maintains patient records accurately, completely, timely and confidentially according to procedures.
12. Serves as a resource person for the oncology program and assists the care team with management of oncologic issues.
13. *Collaborates with staff to provide education for the patient and family according to their needs, abilities, preferences and readiness to learn; assists patients, family and/or staff with problem-solving as indicated.
14. Supports the inpatient and outpatient discharge process for continuity of care and optimal patient outcomes; reviews and revises patient education materials; performs telephone follow-up calls on patients post discharge, at regular intervals.
15. Assists in the identification and reporting of quality issues and contributes to the performance improvement plan for the Oncology Program.
16. May provide staff education at the bedside, in an outpatient setting, in formal settings and to outside agencies as appropriate.
17. Exhibits behaviors and actions which create a high level of patient/ customer satisfaction, which contributes to positive patient/customer relations and reflects respect for the patient's/customer's rights, needs and confidentiality.
18. *Ensures all patient care is provided in accordance with medical center and other regulatory and safety requirements including the Joint Commission, OSHA, state and federal regulations.
19. Participates in a minimum of four (4) cancer program outreach events per year.
20. Champions Multidisciplinary Clinic (MDC) workflows and processes as applicable.
21. Participates in organizational and programmatic strategic planning, regularly updates staff on progress.
22. Regularly seeks educational opportunities and shares with the cancer program team. May provide direct education to staff on occasion.
23. Active member of the cancer program quality team; attending monthly meetings and contributes to agenda items. Works collaboratively with cancer program leadership and the Quality Department and other quality databases to identify areas of opportunity and improve data reporting.
EDUCATION AND EXPERIENCE:
Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Bachelor's degree with Oncology Nurse Navigator - Certified Generalist (ONN-CG), Certified Breast Care Nurse (CBCN) or equivalent certification OR Master's degree required. Previous experience as Oncology Nurse Navigator required.
OTHER CREDENTIALS / CERTIFICATIONS:
Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
Oncology Certification required or obtained with 24 months of hire.

- Record and Report all patient data accurately using the Database Euclid and TDMS
- Administration of prescribed medication as per doctor’s orders
- Disinfect the machines as per protocol
- Education of patient on renal disease aetiology and treatment modes, blood results, support services and dietary restrictions

Job Description
m2m is seeking a highly skilled Registered Nurse on a fixed-term contract to support in delivering Comprehensive Primary Health Care Services at the Ikhwezi Health Centre (Innovation Hub).The Registered Nurse will play a critical role in supporting the implementation of these integrated health services at the Innovation Hub in Ikhwezi. This includes delivering an enhanced package of services aligned with m2m's strategic focus on addressing key health challenges. The primary focus of this role will be health promotion, disease prevention, case finding, screening, and linking individuals to treatment, care, and support for Tuberculosis (TB), Non-Communicable Diseases (NCDs), and Cervical Cancer. The successful candidate will contribute significantly to the effectiveness of m2m’s health interventions, ensuring high-quality care and support for those in need.
Key Performance Areas:
KPA 1: Health education and promotion
- Conduct and supervise health promotion activities focused on disease prevention and healthy lifestyle education.
- Promote awareness and preventive measures for HIV, TB, HPV, cervical cancer, hypertension, and diabetes and provide technical support and mentorship to peers in alignment with DoH guidelines and protocols
- In collaboration with DoH and m2m Peer Mentors, lead community outreach campaigns focusing on engaging the community and ensuring access to preventive services such as HPV vaccination, TB screening, case finding, and NCD screenings.
KPA 2: Primary prevention and screening
- Conduct routine screenings for HPV, cervical cancer, hypertension, and diabetes for clients accessing services at the health facility.

- Ensure timely referral of clients with pre-cancerous lesions, non-communicable diseases, or TB to relevant service points within the health facility and provide follow-up to ensure their continued care.
- Ensure clients adhere to treatment plans and monitor follow-ups for those in care for TB, NCDs, and cervical cancer prevention.
- Support peers ensuring that all referred clients are linked to care and have accessed required services
KPA 4: Relationships management
- Maintain close collaboration with Department of Health clinical staff at the health facility to ensure cohesive service delivery.
- Foster strong working relationships with health facility teams through regular participation in health facility meetings and support outreach campaigns.

Qualifications and Experience
- A minimum qualification of a BCur Nursing degree or equivalent nursing qualification as a Registered Nurse.
- Active registration with the South African Nursing Council (SANC) and DENOSA.
- Primary Health Care Training / APC (Adult Primary Care),NIMART training will be an addedd advantage
- 5-10 years of experience in providing comprehensive Primary Health Care services, including health education, promotion, prevention, and management of non-communicable diseases, TB, cervical cancer, HPV, and immunization.
- Proficient in conducting Pap smears.
- Strong passion for community health work.
- Excellent communication and writing skills.
- Demonstrated ability to problem-solve, make decisions, and work independently with minimal supervision.

Hours: Contracted hours will be agreed upon before starting in the post and will correspond with our respite service which is pre-booked throughout the year. There may also be scope for bank nurses to cover additional shifts on an ad hoc basis.
Shifts: 7:30 am to 2:30 pm and 2:00 pm to 8:30 pm. Night shifts run from 8 pm to 8 am and you may occasionally be asked to cover a night if the regular night team are unavailable.
Your primary role will be to provide high-quality palliative care in true collaboration with our patients plus their family members and those most important to them. This will include holistic assessment, planning, delivery and evaluation of person-centred care plans. You’ll serve as a role model for team members, providing supervision, motivation, support and encouragement. You’ll be working within a team composed largely of volunteers, some of whom have decades of experience; and you’ll need to embrace the Hospice’s ethos and support our commitment to the evolution of a volunteer care delivery model. You will be supported by the Lead Nurse / Deputy Lead Nurse.
